An unannounced visit was conducted to investigate a complaint and annual survey at Heritage Pointe Assisted Living from January 23, 2018 to January 25, 2018 by the Department of Health and Human Services Division of Public Health.

However, deficiencies were found related to failure to evaluate a resident's ability to self-administer medications, expired medication aide certifications, and food safety issues including outdated food, improper dishwasher sanitation, and inadequate hot food temperatures.
