The inspection was conducted as a renewal inspection of the facility's license.
Findings
The inspection found multiple deficiencies related to staff orientation, medical evaluations, fire drills, self-administration assessments, and insulin injection training. The facility submitted a plan of correction which was determined to be fully implemented.

Staff person did not complete training on resident rights, mandatory reporting of abuse and neglect, emergency medical plan, and reporting of reportable incidents within 40 scheduled working hours.
An unannounced fire drill was not held during the month of July 2023.
Resident medical evaluations did not include required information such as height, weight, temperature, immunizations, allergies, and physician's license number.
Resident #2 self-administers medication but has not been assessed by a qualified medical professional regarding ability to self-administer and need for reminders.
Staff Person A's diabetic training expired, yet the staff person obtained a blood glucose reading on a resident.
