An unannounced visit was conducted to investigate a complaint and annual survey at Heritage Ridge Retirement Community from October 10 to October 15, 2013, to assess compliance with licensure regulations and investigate specific allegations.

The facility was found to be in compliance with discharge criteria and protection from misappropriation. However, the facility failed to maintain proper food temperatures and failed to evaluate and notify the physician of Resident 8's blood sugar levels outside established parameters.
