Findings
During a virtual tour of the facility and resident #1's room, it was found that the resident side of the door was not a single motion lock and remained locked until manually unlocked. Staff interview confirmed all resident room doors in the facility have this issue.

Findings
The inspection identified multiple deficiencies including incomplete employee files for two employees, missing documentation for PRN medication administration for a resident, and failure to update a resident's service plan to reflect hospice care.
