Visit Reason
The inspection was conducted as a provisional survey to assess compliance with licensing regulations for the Hill Street Program, a Level I Residential Care Facility.
Findings
The facility was found non-compliant with multiple regulatory requirements including failure to complete background checks for employees at time of hire, lack of written releases for confidential information for residents, failure to provide resident rights documentation, unsafe medication procedures, missing annual physical for a resident, incomplete resident summary sheets, lack of required fire drills, and inadequate sanitation in the bathroom.
