Follow-up survey to determine if orders from the September 18, 2024 survey were corrected.
Findings
The follow-up survey verified that the facility is in substantial compliance with previous orders. However, prior surveys found deficiencies including fire protection hazards, tuberculosis prevention program gaps, emergency preparedness plan deficiencies, fire safety and evacuation plan issues, and physical environment maintenance problems.
