An on-site inspection was completed on July 8, 2008 for the purpose of renewing the license and to process a request from the licensee designee to reduce the facility's capacity from eleven residents to nine residents.
Findings
The facility's capacity was reduced from eleven to nine residents by removing one bed each from bedroom four and bedroom five, which were semi-private rooms. The licensee's request to reduce capacity was approved.
