Visit Reason
The Minnesota Department of Health conducted an initial survey on March 13, 2024, to assess compliance with state licensing statutes for an assisted living facility license.
Findings
The licensee was found to be in substantial compliance with state statutes but had several deficiencies including failure to list the licensed assisted living director as Director of Record, food preparation violations per Minnesota Food Code, incomplete emergency preparedness plan, inadequate fire safety and evacuation drills, and lack of individualized medication management plan for a resident.
