Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for an assisted living facility with dementia care license.
Findings
The facility was found in substantial compliance but had multiple level two violations including failure to prepare and serve food according to Minnesota Food Code, failure to post emergency numbers, failure to maintain fire extinguishers, incomplete fire safety and evacuation plans, failure to provide required written notices for emergency relocations, incomplete training and competency evaluations for unlicensed personnel, incomplete dementia care training, failure to revise service plans to reflect current services, medication administration errors, failure to date insulin pens, incomplete documentation of medication disposition, and incomplete treatment and therapy management plans and documentation.
