The California Department of Social Services Community Care Licensing Division conducted three unannounced investigations of the 740‑type residential facility **Ivy Park at Claremont** (Facility Number 198603729) in August 2024, March 2025, and April 2025. Each visit followed the same procedural framework: the evaluator, Kimberly Ramirez, met with staff, reviewed records, observed the premises, and interviewed residents where possible.
During the August 2024 visit, a resident complaint that staff member S10 shouted and refused to assist residents was found to be substantiated. Five of nine staff interviews and three written statements corroborated the allegation, and S10 had resigned on the day of the visit. The evaluator cited a type B deficiency under CCR 87468.1(a)(1) for failing to treat residents with dignity. The facility was required to retrain staff on this regulation by 8 / 29 / 24 and submit proof of training. Only one resident interview refuted the claim, but the evidence met the preponderance standard.
The March 2025 investigation addressed a separate complaint of improper infection‑control practices during an active COVID‑19 outbreak. Staff had not suspended communal dining or required masks in common areas despite a CDPH‑issued surveillance order and a county violation. The evaluator found the allegation substantiated, citing a type A deficiency under CCR 87465(a)(9) for failing to maintain infection‑control measures during an epidemic. The administrator was ordered to re‑train staff and certify compliance by 3 / 12 / 25, with proof due 3 / 25 / 25.
The April 2025 inspection concerned pest infestation. After reviewing staff reports and conducting a plant tour, the evaluator determined the allegation unsubstantiated. No evidence of significant infestation was found, and the facility was not cited for any deficiencies. The report closed with a brief exit interview and notice of appeal rights.
