IVY PARK AT SEVEN OAKS, a 164‑bed residential care facility for the elderly in Bakersfield, underwent a series of regulatory inspections during 2022–2023. The first visit on 12/09/2022 was a Component II verification for a change‑in‑ownership application. Administrator Krystal Jenkins confirmed full knowledge of licensing laws, and the report noted the facility was in compliance at that time. A follow‑up Pre‑Licensing Component III inspection on 12/28/2022, conducted by Licensing Program Analyst M. Yang, examined the facility’s physical plant, health‑care procedures, and staffing records. While the facility met most requirements, the evaluator identified several safety and medication‑management deficiencies, including the need for non‑skid mats in all shower areas, locked storage for medications, cleaning chemicals, and sharps. An exit interview outlined corrective actions, and a plan of correction was to be submitted within 30 days.

On 01/09/2023, a third unannounced visit confirmed that the corrective actions taken after the December inspection had been implemented. Medications and sharps were secured, cleaning chemicals were locked, and the facility had met all pre‑licensing requirements. Licensing Program Manager was notified that a license could be issued pending final review. However, the same analyst returned in July 2023 for two separate case‑management deficiency visits. The first, on 07/18/2023, addressed an incident where Resident R1 went AWOL on 07/10/23, evidencing inadequate supervision and prompting a citation under 87413(a)(2). A plan of correction required an AWOL‑training module and attendance roster by 08/01/23. The second visit on 07/24/2023 followed a medication error that occurred on 07/20/23, resulting in a citation under 87465(a)(5) for failure to assist residents in medication administration. In both instances, the analyst emphasized the facility’s continued need to enhance resident safety protocols and reinforce medication‑management procedures.

Across all inspections, a consistent pattern emerged: while the facility largely satisfies licensing standards, recurring deficiencies in resident supervision, medication and hazardous‑material storage present ongoing compliance risks. The repeated requirement for corrective action and the documentation of deficiencies suggest that IVY PARK AT SEVEN OAKS must strengthen its oversight mechanisms, staff training, and internal safety procedures to prevent future violations and ensure a consistently safe environment for its residents.
