The California Department of Social Services Community Care Licensing Division conducted a pre‑licensing inspection of Ivy Park at Studio City on February 6, 2025. The four‑story, 121‑bed facility, managed by Executive Director Faraz (Dan) Kashani, was evaluated by LPAs Nadia Shahbazian and Gary Tan. The inspectors reviewed fire safety, resident accommodations, food service, and medication procedures, finding the site ready for licensure under Title 22 regulations.
Fire safety checks confirmed that hot‑water temperatures were within the acceptable range (115.2–117.9 °F) and grab bars were properly installed in every bathroom. Signal systems were operational, activated by pull cords in each room and by exit doors, with evacuation chairs positioned by stairwells. The facility’s two elevators and dual exit doors per floor met required safety standards. The inspectors also noted that no body of water or swimming pool was present, eliminating aquatic hazards.
In the residential and dining areas, all sharp knives, cutlery, kitchen cleaners, chemicals, and laundry detergents were locked away as mandated. Kitchen equipment, refrigeration, and storage complied with Title 22 specifications, and food supplies were adequate and properly stored. Medication rooms were secure, with medication cards locked on several floors, and the wellness center’s medical procedures were current. First‑aid kits were complete, and the overall physical environment met Division 6 of the CA Code of Regulations.
The inspection concluded that Ivy Park at Studio City’s physical environment is compliant and ready for licensure. No deficiencies were reported, and the facility’s staff met all regulatory expectations. The report, signed by the LPAs and the facility representative, will be made available for public review for three years as required.
