Facility did not utilize the Maine Background Check Center to obtain comprehensive background checks for 3 staff with direct resident access.
Administrator failed to comply with regulations requiring registration with MBCC and obtaining background checks prior to hiring staff.
Dishwashing sanitation standards were not met; no sanitation cycle observed on mechanical dishwasher and manual dishwashing methods did not ensure sanitization.
Facility failed to show evidence of routine maintenance and housekeeping; dust accumulation on bathroom vents and worn countertop surface observed.
A bathing room was not equipped with a call bell as required for safety and privacy.
