Follow-up survey to determine correction of orders from the survey completed on March 27, 2025.

The follow-up survey verified that the facility is in substantial compliance.

Licensee was not legally responsible for the management, control, and operation of the whole facility where assisted living and skilled nursing services were provided in the same building.
Failed to comply with Minnesota State Fire Code including delayed egress door locking systems without proper deactivation devices, fire-resistant rated door not closing and latching automatically, and unsecured oxygen cylinders.
Failed to provide interconnected smoke alarms meeting requirements for existing power supply inside sleeping rooms.
Failed to develop and maintain a fire safety and evacuation plan including specific fire protection actions for residents.
Failed to ensure registered nurse conducted direct supervision of staff performing delegated tasks within 30 days of providing services for one unlicensed personnel.
Failed to ensure employees received orientation including all required content for two employees under transition to a new management company.
