Findings
The June 19, 2023 annual compliance inspection found nine deficiencies related to staff training, notification of manager changes, documentation provision, resident health records, medication storage, disaster plan reviews, and drills.
Deficiencies (9)
Description
36-420.01. Health care institutions; fall prevention and fall recovery; training programs; definition: Failed to develop and administer a training program for all staff regarding fall prevention and fall recovery.
A. A governing authority shall: 7. Except as provided in subsection (A)(6), notify the Department according to A.R.S. § 36-425(I) when there is a change in the manager and identify the name and qualifications of the new manager;: Failed to notify the Department of manager changes as required.
E. A manager shall ensure that, unless otherwise stated: 1. Documentation required by this Article is provided to the Department within two hours after a Department request; and: Failed to provide required documentation to the Department within two hours after request.
A. Except as provided in R9-10-808(B)(2), a manager shall ensure that a resident provides evidence of freedom from infectious tuberculosis: 1. Before or within seven calendar days after the resident's date of occupancy, and 2. As specified in R9-10-113.: Failed to ensure resident provided evidence of freedom from infectious tuberculosis within required timeframe.
B. A manager shall ensure that before or at the time of acceptance of an individual, the individual submits documentation that is dated within 90 calendar days before the individual is accepted by an assisted living facility and: 1. If an individual is requesting or is expected to receive supervisory care services, personal care services, or directed care services: a. Includes whether the individual requires: i. Continuous medical services, ii. Continuous or intermittent nursing services, or iii. Restraints; and b. Is dated and signed by a: i. Physician, ii. Registered nurse practitioner, iii. Registered nurse, or iv. Physician assistant; and 2. If an individual is requesting or is expected to receive behavioral health services, other than behavioral care, in addition to supervisory care services, personal care services, or directed care services from an assisted living facility: a. Includes whether the individual requires continuous behavioral health services, and b. Is signed and dated by a behavioral health professional.: Failed to ensure required documentation dated within 90 days and signed by appropriate medical professional was submitted before or at acceptance.
F. When medication is stored by an assisted living facility, a manager shall ensure that: 1. Medication is stored in a separate locked room, closet, cabinet, or self-contained unit used only for medication storage;: Failed to ensure medication was stored in a separate locked area; medications were observed unlocked and accessible.
A. A manager shall ensure that: 2. The disaster plan required in subsection (A)(1) is reviewed at least once every 12 months;: Failed to ensure disaster plan was reviewed at least once every 12 months.
A. A manager shall ensure that: 4. A disaster drill for employees is conducted on each shift at least once every three months and documented;: Failed to ensure disaster drills were conducted on each shift at least once every three months.
A. A manager shall ensure that: 5. An evacuation drill for employees and residents: a. Is conducted at least once every six months; and: Failed to ensure evacuation drills were conducted at least once every six months.
