Visit Reason
The inspection was conducted as a result of a State Licensure annual grading and complaint investigation survey at the facility.
Findings
The facility was found to have multiple deficiencies related to caregiver qualifications, medication management training, elder abuse training, tuberculosis screening, and medication storage. Several complaints were investigated with some substantiated regarding incomplete employee records and medication management training.

The inspection was a mandatory re-grading State Licensure survey conducted to assess compliance with Nevada Administrative Code Chapter 449 for Residential Facility for Groups.

The inspection was conducted as an off-site change of ownership State Licensure survey for Kings Row Residence, requesting approval for 10 Residential Facility for Group beds for elderly and disabled persons and/or persons with mental illness, chronic illness, or intellectual disabilities.
Findings
The facility policies and protocols were reviewed, and the application was approved for 10 beds. No regulatory deficiencies were identified and no further action was necessary.
