Visit Reason
The Adult Care Licensure Section conducted an initial survey of Kingswood Reserve on November 5-6, 2024, to assess compliance with state regulations for adult care facilities.

Findings
The facility was found deficient in completing resident care plans within 30 days of admission, timely Licensed Health Professional Support (LHPS) evaluations, proper medication administration according to orders, and secure medication storage. Specific issues included delayed care plan completion and signatures, missed or late LHPS evaluations for three sampled residents, administration of medication outside prescribed blood pressure parameters, and an unlocked medication cart left unattended.
