A relicensure survey with complaint investigations was conducted at Kyrie Assisted Living from November 18 through November 20, 2025, to investigate multiple complaints regarding staffing, resident care, supervision, discharge processes, and facility environment.

The investigations included observations, interviews, and record reviews related to staffing adequacy, resident safety, care planning, discharge procedures, and facility cleanliness. No deficiencies were cited during the survey.

Multiple complaints were investigated including inadequate staffing, failure to involve legal guardians in care planning, inadequate supervision, failure to provide discharge notices and paperwork, failure to timely assess and intervene for resident condition changes, and failure to maintain a safe, clean, homelike environment free of pests. The investigations were unannounced and included a sample of 7 residents. No deficiencies were found.
