Visit Reason
An unannounced verification survey was conducted at Ladysmith Adult Day Services to assess compliance with Wisconsin Administrative Code 105.14 and federal home and community based settings (HCBS) rule 42 CFR § 441.301.

Findings
Ladysmith Adult Day Services was found to be in compliance with all applicable regulations with no deficiencies identified during the survey.

Visit Reason
An unannounced onsite recertification survey was conducted at Ladysmith Adult Day Services on 4/24/2024 to assess compliance with Wisconsin Administrative Code 105.14 and related caregiver regulations.

Findings
The facility was found out of compliance with three deficiencies related to continuing education for staff, medication administration and storage, and caregiver background checks. No deficiencies were found related to federal home and community based settings (HCBS) rule compliance.

Deficiencies (3)
Failed to provide the required 10 hours of annual continuing education for staff in topics including Participant Rights, Preventing and Reporting Abuse, and Emergency/Evacuation Procedures in 3 of 3 staff files reviewed.
Failed to have a participant's prescription medication stored in its original, labeled container; medication was stored in a plastic pill organizer instead.
Did not ensure that all staff had caregiver background checks completed every 4 years; one staff file had a background check dated over 4 years ago.
