The Adult Care Licensure Section conducted an initial survey of Lakepark Family Care Home on December 18, 2019.
Findings
The facility failed to meet several regulatory requirements related to staff qualifications and food safety. Specifically, one of three sampled staff (Staff C) lacked documentation for tuberculosis testing, Health Care Personnel Registry check, criminal background check, competency validation by a licensed health professional, medication clinical skills validation, and controlled substance screening. Additionally, the facility failed to label and date opened food items in the refrigerator to prevent contamination.
