Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for The Landmark of Fridley assisted living facility.
Findings
The survey identified multiple violations including failure to develop and implement a written staffing plan with biannual evaluations, failure to ensure food was prepared and served according to Minnesota Food Code, failure to maintain a tuberculosis prevention and control program with proper employee screening, and failure to ensure proper medication administration by unlicensed personnel.
