Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for the Assisted Living Facility license.
Findings
The licensee was found to be in substantial compliance but had violations including failure to prepare and serve food according to Minnesota Food Code, failure to maintain an infection control program consistent with accepted standards, and failure to securely store medications. The food code violations were widespread with a level two severity, infection control and medication storage violations were level two with isolated and widespread scope respectively.
