Visit Reason
The Minnesota Department of Health conducted a full survey of Lindenwood Assisted Living on May 21, 2025, to evaluate and assess compliance with state licensing statutes.
Findings
The facility was found to be in substantial compliance but had violations related to infection control, fire protection and physical environment, and service plan implementation. Two level two violations were issued for improper hand hygiene and fire safety code noncompliance, and one level three deficiency was noted for incomplete service plan updates.
