Visit Reason
The Adult Care Licensure Section conducted an annual survey of Vintage Gardens on 04/25/24 - 04/26/24 with an exit conference via telephone on 04/26/24.
Findings
The facility was found deficient in multiple areas including failure to conduct required Health Care Personnel Registry checks prior to hiring, failure to ensure tuberculosis testing upon admission for residents, failure to have physicians sign and date care plans within 15 days of assessment, medication administration errors including failure to administer ordered antibiotics and improper continuation of discontinued medications, and failure to notify the county Department of Social Services of resident accidents requiring emergency evaluation.

Visit Reason
The Adult Care Licensure Section conducted an attempted annual survey of the facility on July 28, 2021.
Findings
Upon arrival, the facility was found to be under renovation with no residents or staff present. Two of the previous three residents had been transferred to other facilities and one discharged to family. The administrator planned to begin reopening the facility in early August 2021.
