Findings

Description
36-422. Application for license; notification of proposed change in status; joint licenses; definitions - Failed to notify the department within thirty days after any change regarding a controlling person and provide required information.
A. A governing authority shall: 7. Except as provided in subsection (A)(6), notify the Department according to A.R.S. § 36-425(I) when there is a change in the manager and identify the name and qualifications of the new manager; - Failed to notify the Department of a change in the facility's manager.
A. A manager shall ensure that: 7. Documentation is maintained for at least 12 months after the last date on the documentation of the caregivers and assistant caregivers working each day, including the hours worked by each; - Failed to maintain documentation of caregivers and assistant caregivers working each day including hours worked.
A. Except as provided in R9-10-808(B)(2), a manager shall ensure that a resident provides evidence of freedom from infectious tuberculosis: 1. Before or within seven calendar days after the resident's date of occupancy, and 2. As specified in R9-10-113. - Failed to ensure residents provided evidence of freedom from infectious tuberculosis.
A. Except as required in subsection (B), a manager shall ensure that a resident has a written service plan that: 3.c. The amount, type, and frequency of assisted living services being provided to the resident, including medication administration or assistance in the self-administration of medication; - Failed to include amount and frequency of assisted living services in resident's service plan.
C. A manager shall ensure that: 1.g. Documents the services provided in the resident's medical record; and - Failed to ensure caregiver documented services provided in resident's medical record.
C. A manager shall ensure that a resident's medical record contains: 12. A medication order from a medical practitioner for each medication that is administered to the resident or for which the resident receives assistance in the self-administration of the medication; - Failed to ensure resident's medical record contained a medication order for administered medication.
B. If an assisted living facility provides medication administration, a manager shall ensure that: 3.b. A medication administered to a resident: Is administered in compliance with a medication order, and - Failed to ensure medication was administered in compliance with a medication order.
D. When a resident has an accident, emergency, or injury that results in the resident needing medical services, a manager shall ensure that a caregiver or an assistant caregiver: 2.a-f. Documents date/time, description, observers, actions taken, notifications, and prevention actions; - Failed to ensure documentation of incident details and prevention actions in resident's medical record.
