Visit Reason
Initial survey completed on 08/30/2024 for state requirements of NMAC 8.370.14, Regulations for Assisted Living Facilities for Adults.
Findings
The facility was found deficient in kitchen sanitation including dirty ceilings, floors, and ceiling tiles with holes and stains; unsecured hazardous chemicals and paints stored in accessible areas; unsecured mechanical room with combustibles and gas cylinders; damaged and unsecured ceiling tiles throughout the facility; damaged and missing window screens and broken window frames; and unsecured hazardous gas fired equipment room.

Facility kitchen ceilings were dirty, stained, with holes and perforations allowing contamination of food.
Kitchen floors were dirty with grease buildup and food particles.
Chemicals, paints, and cleaning supplies were stored unsecured and accessible to residents in the activity room.
Ceiling tiles in dining room, common areas, and hallways were not secured, had perforations and holes.
Mechanical room door was unlocked and open, containing combustibles, gas cylinders, and blocked access to circulation pumps.
Windows and window screens were damaged, broken, missing or did not close or lock properly, allowing entry of dirt, bugs, leaves, and intruders.
