Visit Reason
The Adult Care Licensure Section conducted an initial survey on November 20 and 21, 2024 to assess compliance with licensing requirements for Lynndale Senior Living Home.
Findings
The survey identified multiple deficiencies including failure to complete criminal background checks for staff, incomplete or outdated medical examinations for residents, lack of timely physician signatures on resident care plans, inaccuracies and omissions in medication administration records, and absence of required quarterly medication reviews by a licensed pharmacist or healthcare provider.
