Multiple deficiencies were identified including uncovered trash receptacles, missing fitted sheets on a resident's bed, lack of individual toiletry items, missing toilet paper in a bathroom, incorrect fire safety evacuation diagrams, missing annual fire extinguisher inspection documentation, incorrect exit signage, and medication labeling and administration errors. All deficiencies had plans of correction accepted and were implemented by 03/22/2024.
