Findings

Description
A. A governing authority shall: 7. Except as provided in subsection (A)(6), notify the Department according to A.R.S. § 36-425(I) when there is a change in the manager and identify the name and qualifications of the new manager; - The governing authority failed to notify the Department of the new manager's name and qualifications.
E. A manager shall ensure that: 1. A bell, intercom, or other mechanical means to alert employees to a resident's needs or emergencies is available in a bedroom being used by a resident receiving directed care services; or - Failed to ensure such alert systems were available in two resident bedrooms.
F. A manager of an assisted living facility authorized to provide directed care services shall ensure that: 1. Policies and procedures are established, documented, and implemented that ensure the safety of a resident who may wander; - Failed to implement policies and procedures to ensure safety of wandering residents; gates were not locked.
A. A manager shall ensure that: 12. Combustible or flammable liquids and hazardous materials stored by the assisted living facility are stored in the original labeled containers or safety containers in a locked area inaccessible to residents; - Failed to ensure hazardous materials were stored in a locked area inaccessible to residents.
