C0240 - Resident Services Meals, Food Sanitation Rule: Failed to maintain a clean and sanitary kitchen with multiple areas needing cleaning or repair including grease buildup, dust accumulation, improper storage of chemicals, and ice buildup in freezer.
C0270 - Change of Condition and Monitoring: Failed to ensure short-term changes of condition were monitored until resolution for sampled residents.
C0363 - Acuity Based Staffing Tool - Updates & Staffing Plan: Failed to update acuity-based staffing tool no less than quarterly for sampled residents.
