Visit Reason
The Minnesota Department of Health conducted a full survey on June 3, 2025, to evaluate and assess compliance with state licensing statutes for Meadow Ridge Senior Living, an assisted living facility with dementia care license.
Findings
The survey identified multiple deficiencies including food safety violations, infection control issues, tuberculosis screening noncompliance, fire safety code violations, incomplete fire safety and evacuation plans and training, incomplete staff orientation and annual training, and late resident reassessments.
