However, on June 10, Ibo revisited the facility unannounced to conduct a records review. The review uncovered an elopement incident involving resident R1, who was found in the parking lot on May 26 and later returned. The facility had failed to file the required incident report with CCLD within the mandated seven‑day period. Additionally, a staffing issue was identified: employee S2 had begun working in April but was not correctly associated with the facility in the licensing records. Both deficiencies were cited under California Code of Regulations sections related to supervision and reporting requirements, with a plan of correction deadline of June 17. Failure to correct these deficiencies could result in civil penalties.
