The inspection was conducted as a renewal inspection of the facility license.
Findings
The inspection identified deficiencies including lack of documentation for disposition of personal possessions in resident files, incomplete medication destruction records, missing rabies vaccination documentation for the facility's cat, and a kitchen garbage container being very full without a lid.

Description
Resident files lacked documentation of disposition of personal possessions for Residents #2, 4, and 5.
Medication records showed controlled medications destroyed but no documentation for other medications destroyed or returned upon resident transfer or discharge for Residents #3, 4, and 5.
Pet files showed the only cat in the facility had no documentation of a rabies vaccination since 01/02/12.
Kitchen garbage container was very full with no lid on it.

The inspection identified deficiencies including outdated policy and procedure manual review, incomplete medication administration records lacking reasons for use, missing oxygen use warning signs on residents' doors, and uncovered garbage cans in laundry rooms.

Description
The last documented review of the Policy and Procedure manual was 1/28/2014, indicating it was outdated.
Medication Administration Records for 11 residents did not include a reason for use on all medications.
No 'No Smoking, Oxygen in Use' sign posted on the doors of residents #4 and #12 who utilize oxygen.
Garbage cans in both laundry rooms did not have lids and were not covered.
