Monte Vista Village Senior Living, a 740‑type residential care facility located at 2211 Massachusetts Avenue, Lemon Grove, California, has undergone a series of Department of Social Services inspections over the past three years. The 2022 pre‑licensing review in January highlighted the facility’s adherence to health‑and‑safety standards—properly furnished rooms, functional fire suppression equipment, and adequate kitchen and activity spaces were all in order. An administrative lapse was noted, however: the Resident Services Director’s certification had expired on October 30, 2021, prompting a reminder for renewal. The August 2022 office visit, conducted under a COMP II interview, confirmed that the then‑administrator, Susan Zanca‑Brown, understood the applicable Title 22 regulations and that the license application’s change of ownership had been properly documented.

A shift in leadership occurred by February 2023, when Paul Markovich had assumed the administrator role. During an unannounced case‑management visit that month, the licensing analyst provided guidance on updating the administrator’s name on the state’s records but did not identify any deficiencies. The next significant event was an unannounced case‑management/incident visit on September 8, 2023, prompted by a self‑reported medication‑administration error. The incident involved Resident #1 receiving medication intended for Resident #2 due to the medication technician’s failure to use the labeled plastic transport containers. While the resident’s health was not seriously compromised and the error was corrected promptly, the facility was cited for failing to assist a resident with self‑administered medication, a Title 22 violation. A plan of correction was developed, and the staff member responsible was removed from medication pass duties and retrained. Two technical violations regarding reporting were also issued.

The January 12, 2024 one‑year inspection, conducted unannounced by Amy Rodgers, confirmed continued compliance across a broad array of operational domains—fire safety, resident care, and environmental standards. The facility houses 219 ambulatory residents, including those with limited mobility and a hospice waiver group. While no new deficiencies were noted, the review reiterated the importance of ongoing staff training, especially in medication management, and reinforced the facility’s commitment to maintaining the standards required by the California Department of Social Services Community Care Licensing Division.
