Morningstar Memory Care at San Tomas (Facility Number 435202874) is a 740‑type residential care facility with a capacity of 82 residents. An initial office visit on 5 May 2023 confirmed that administrator Ignacio Lopez and licensee designee Phil Altman had completed COMP II and understood all applicable licensing regulations, and the facility’s readiness for opening was documented. An unannounced pre‑licensing inspection on 24 May 2023 found the building complete with functional fire safety equipment, properly stored medication and cleaning supplies, and no residents on the premises. The visit noted adequate lighting, temperature control, and an operational elevator, and no deficiencies were cited.
A serious resident‑staff incident reported on 19 July 2023—where staff member S1 pinched and twisted resident R1’s nose after an altercation—prompted a case‑management inspection on 1 August 2023.
