Visit Reason
The Minnesota Department of Health completed a survey to evaluate and assess compliance with state licensing statutes for an assisted living facility with dementia care license.
Findings
The licensee was found to be in substantial compliance but had multiple violations including infection control, food service, abuse prevention plans, tuberculosis screening, emergency preparedness, fire safety, resident record documentation, service plan completeness, medication management, treatment and therapy management, and activity planning.
