Visit Reason
The inspection was conducted as a partial extended survey following identification of Immediate Jeopardy related to nursing staff competencies and wound care management.

Findings
The facility failed to ensure nursing staff possessed the necessary competencies to safely care for residents, resulting in an Immediate Jeopardy related to improper handling of a resident's fall injury and failure to adhere to physician wound care orders. The Immediate Jeopardy was removed after a validated Removal Plan, but ongoing noncompliance was noted at a lower severity level.

The inspection was conducted to assess compliance with regulatory requirements related to resident care, safety, and facility environment at Nhc Healthcare, Franklin.

Findings
The facility was found deficient in multiple areas including failure to hold quarterly care plan meetings, failure to notify resident representatives of significant changes, inadequate housekeeping and maintenance leading to unsanitary conditions, failure to implement nutritional interventions, incomplete skin assessment documentation, failure to provide appropriate catheter care and documentation, and failure to monitor oxygen administration and equipment maintenance.
