Follow-up survey conducted to determine correction of orders found on the survey completed on March 27, 2025.
Findings
The facility had corrected all state correction orders issued pursuant to the March 27, 2025 survey. However, a violation related to failure to comply with state and local governing laws regarding construction permits and plan review was identified.
