In each instance, the California Department of Social Services Community Care Licensing Division received complaints alleging lapses in health and safety—from a reported gastrointestinal outbreak and unsanitary conditions in September 2022, to unexplained fractures and bruising in October 2022, and finally to odor, hygiene, and general upkeep concerns in October 2024. The 10/21/2022 review of fractures and bruising likewise yielded insufficient evidence to confirm any violation, with medical records indicating that the injuries were consistent with the resident’s underlying osteopenia and could not be linked to care. The October 2024 inspection found the facility clean, odor‑free, and well‑maintained, and staff confirmed that hygiene protocols were consistently applied.
