All visits were conducted under the direction of Administrator Tom Park and consistently documented that the facility remained in compliance with Title 22 regulations. The 08/20/2021 office visit was a COMP II telephone check that confirmed the administrator’s knowledge of licensing duties, staff qualifications, and program policies. The 09/23/2021 pre‑licensing inspection, an unannounced visit, confirmed that the transition of ownership from Facility #197609905 to #197610184 had been completed, that residents were present and well‑maintained, and that fire and safety systems met state standards.

A 05/24/2022 case‑management review focused on staff roster integrity. The analyst verified the removal of a former MedTech who was no longer employed, updated personnel records, and cleared any potential compliance risk. During the same visit the physical plant showed no health or safety issues. The 09/21/2022 annual required inspection examined infection control, food safety, fire protection, resident and staff records, and medication documentation. Staff wore appropriate PPE, the kitchen was sanitary with adequate food supplies, smoke detectors and fire alarms were functional, and all resident rooms and common areas were properly furnished and cleaned. The analyst noted that all resident and staff files were current and that medication records were accurate. No deficiencies or violations were reported in any of the visits.
