Oakmont of Whittier (Facility No. 198603479) was the subject of three unannounced Community Care Licensing investigations conducted by Evaluator Angelica Rea. The first visit on 18 November 2022 addressed a range of concerns—claims that staff were not following a resident’s care plan and that a resident was being illegally evicted. Interviews with Administrator Janette Hill, staff, and the resident, together with a review of the resident’s file and eviction notice, revealed no clear evidence of non‑compliance; the allegations were therefore marked **unsubstantiated** and no deficiencies were cited.

In a second visit on 25 July 2023, the agency examined an allegation that staff unlawfully evicted a resident while the resident was still in care. Records showed that the resident had been discharged from a skilled nursing facility and that, upon receiving a call from the Administrator and Health Services Director, the facility declined to accept the resident back on 6 July 2023. Because the facility failed to provide the required thirty‑day written notice and did not follow Title 22 eviction procedures, the allegation was found **substantiated**. Deficiencies under California Health and Safety Code § 87224(a) were cited, and a plan of correction requiring proper eviction training and documentation was imposed.

A third investigation on 21 November 2023 focused on a single allegation that staff did not dispense medications to a resident as prescribed. Review of the resident’s medication list, discharge documents, and a special incident report revealed that the medication list had not been updated after the resident’s return from a prolonged hospitalization. The agency found the allegation **substantiated** and cited deficiencies in medication management under Title 22. A plan of correction was issued, mandating immediate updates to medication records and reporting of all incidents to the agency.

Across all visits, the pattern that emerges is repeated concerns about resident‑management procedures—particularly eviction protocols and medication updates—while care‑plan adherence claims were consistently unsubstantiated. Oakmont of Whittier has responded to each substantiated finding with corrective plans, but the persistence of similar issues suggests that the facility’s administrative and clinical processes require sustained oversight to ensure full compliance with California regulations.
