Deficiencies under California Health and Safety Code § 87224(a) were cited, and a plan of correction requiring proper eviction training and documentation was imposed.
A third investigation on 21 November 2023 focused on a single allegation that staff did not dispense medications to a resident as prescribed. Review of the resident’s medication list, discharge documents, and a special incident report revealed that the medication list had not been updated after the resident’s return from a prolonged hospitalization. The agency found the allegation **substantiated** and cited deficiencies in medication management under Title 22. A plan of correction was issued, mandating immediate updates to medication records and reporting of all incidents to the agency.
Across all visits, the pattern that emerges is repeated concerns about resident‑management procedures—particularly eviction protocols and medication updates—while care‑plan adherence claims were consistently unsubstantiated.
