The purpose of this survey was to investigate intake #GA50004440 and #GA50004727, with the investigation beginning on 8/27/25 and completed on 8/28/25.

Complaint Details
The investigation was initiated based on intake #GA50004440 and #GA50004727. The investigation included review of personnel files and interviews confirming lack of required certifications and training for staff.
Findings
The facility failed to show evidence of current certification in emergency first aid, CPR, fire and life safety training, and emergency evacuation procedures for 5 sampled staff. Additionally, new staff had not received comprehensive training on emergency evacuation and use of the evacuation chair. The elevator was non-functional for a period, causing evacuation challenges for residents, including bedbound and wheelchair users.
