OCEAN HILLS ASSISTED LIVING & MEMORY CARE, located at 4500 Cannon Rd, Oceanside, CA, received a series of unannounced case‑management visits from the California Department of Social Services, Community Care Licensing Division (CCLD) between July 2021 and June 2022. Early inspections in July and August 2021, conducted by Licensing Program Analyst Kristina Ryan, focused on technical assistance and evaluation of the facility’s COVID‑19 mitigation plan. Both visits found no deficiencies. A required one‑year review on April 26, 2022, led by Rebecca Ruiz, also concluded without findings, indicating continued compliance with infection‑control protocols.

During the November 19, 2021 visit, the agency followed up on medication‑error reports involving Residents 1, 2, and an additional incident involving Resident 3. The agency cited a deficiency under Title 22, Division 6, Chapter 8 for failure to administer medications as prescribed, a risk that could jeopardize resident safety. A second deficiency was issued on December 8, 2021, for the facility’s failure to file an incident report within the required seven‑day period after a serious injury (a witnessed fall). Both incidents prompted the issuance of a Plan of Correction (POC) and highlighted the need for improved documentation and staff training.
Across the inspections, the facility consistently engaged with CCLD staff, received copies of the reports and licensee rights documents, and participated in exit interviews. While the infection‑control measures remained robust, the medication‑management and incident‑reporting deficiencies underscored recurring challenges in operational compliance. The corrective actions and ongoing monitoring outlined in the reports suggest that OCEAN HILLS is taking steps to address these gaps, but the pattern of deficiencies indicates that continued oversight will be necessary to ensure sustained compliance and resident safety.
