Findings
Across multiple complaint investigations, the facility exhibited numerous deficiencies including failure to maintain proper documentation, incomplete or missing service plans, medication administration errors, inadequate investigation of abuse allegations, and failure to ensure residents were treated with dignity and respect. Description
R9-10-807.H.1-5. Residency and Residency Agreements: Manager failed to ensure written notice of termination included contact information for the State Long-Term Care Ombudsman.
R9-10-803.J.1-6. Administration: Manager failed to ensure investigation and incident report for allegation of sexual assault.
R9-10-810.B.1. Resident Rights: Manager failed to ensure residents were treated with dignity, respect, and consideration.
