Visit Reason
The Minnesota Department of Health conducted an initial survey to assess compliance with state licensing statutes for an assisted living facility license.
Findings
The licensee was found in substantial compliance but had several deficiencies including failure to list the licensed assisted living director as Director of Record, failure to prepare and serve food according to Minnesota Food Code, lack of an individual abuse prevention plan for one resident, incomplete tuberculosis prevention program for an employee, inadequate fire safety and evacuation plans and training, and failure to provide required emergency relocation notice for one resident.
