An unannounced desk review Verification Visit survey was conducted as a follow-up to the Recertification survey on 07/17/2023 at Options for Community Growth Inc, an Adult Day Care Center.
Findings
Options for Community Growth Inc. was found to be in compliance with Wisconsin Administrative Code DHS 105.14 for Adult Day Care Centers. The previous citations have been corrected.

Visit Reason
An unannounced onsite recertification survey was conducted on 7/17/23 at Options for Community Growth, an Adult Day Care Center, to assess compliance with Wisconsin Administrative Code DHS 105.14 for Adult Day Care Centers.
Findings
The facility was found out of compliance with food safety regulations including failure to dispose of expired food, improper food storage on the floor, water heater temperature issues, and lack of annual tornado drills documentation.
Deficiencies (4)
Failed to dispose of expired food in 2 of 2 refrigerators, including expired salad dressing, mayonnaise, orange juice, sour cream, cheese, eggs, canned soup, and canned fruits with expiration dates from 2019-2022.
Failed to store food off the floor in 1 of 2 kitchens; canned goods and lunch boxes were observed on the floor near the refrigerator.
Water heaters were not set at 140 degrees Fahrenheit and water temperature at faucets was outside the required 110-115 degrees Fahrenheit range due to lack of mixing valves on 2 of 2 water heaters inspected.
Did not conduct annual tornado drills in 2022 and 2023 as required; no documentation of tornado drills was provided.
