The California Department of Social Services’ Community Care Licensing Division conducted a series of unannounced case‑management visits to PACIFICA SENIOR LIVING MERCED (facility number 247206921). Each interaction was carried out by Licensing Program Analyst Lady Cabrera via telephone due to COVID‑19 precautions. The first two visits (January 28 and March 4, 2021) addressed an unusual incident/injury report filed on January 21, 2020. In the March visit, the agency documented that the licensee failed to meet California Code of Regulations, Title 22, § 87468.1(a)(3) concerning residents’ personal rights, and a deficiency was cited on the LIC 809‑D form. The administrator, Tyler Wilds, received an exit interview and the corresponding appeal rights documentation.

A subsequent March 25, 2021 visit revealed a medication error: staff administered a double dose to Resident R1, resulting in a second deficiency under § 87465(c)(2) for incidental medical care. The violation was noted as a repeat offense within the past twelve months (the first occurring on November 17, 2020), leading to a $250 civil penalty. The report documented staff retraining and monitoring of the resident, and a plan of correction with a due date of April 9, 2021.

During April 23, 2021, the agency reached out to Memory Care Director Tracy Seibert regarding an unusual incident reported on March 23, 2021 and a medication error. The agency requested several documents, including admission agreements, physician orders, medication lists, and staff contact information, to be submitted by April 27, 2021. A similar follow‑up on April 18, 2021 addressed a change of resident condition reported on March 18, 2021, with the same documentation request.

The final visit on May 5, 2021 highlighted another medication lapse: Resident R1 missed a morning dose because the medication was misplaced. Staff was written up and retrained. Throughout these encounters, the facility consistently provided requested documentation and was reminded of its appeal rights. The pattern indicates ongoing challenges in medication management and adherence to personal‑rights regulations, prompting repeated deficiencies and corrective action requirements.
