In the 06/18/2021 and 07/20/2022 annual infection‑control inspections, the licensing program analyst (LPA) Katie Brown observed comprehensive compliance with COVID‑19 mitigation plans, including daily symptom screening, vaccination, testing, PPE storage, and well‑posted hygiene signage. In February and November 2022, case‑management visits focused on incident reports. The February 25 visit addressed a parking‑lot altercation involving a former staff member (S1), a current staff member (S2), and resident R1. The incident, which resulted in no injuries, prompted the filing of a police report and a special incident report (SIR), but no citations were issued. The November 21 visit followed two separate SIRs: a fall of resident R2 on 11/18 and an injury of resident R1 following a hospital stay. Interviews and record reviews were completed, and no citations were issued, though the facility was reminded of its SOC 341 reporting requirements.

The March 15, 2023 case‑management visit marked a shift. LPA Brown reviewed three SIRs—an altercation of resident R2 with staff (S1) on 1/4/23, a fall of resident R3 on 11/12/22, and a medication error involving resident R1 on 1/17/23. Deficiencies were cited under Section C for personnel training, specifically the lack of a trained Med Tech for safe medication administration. A plan of correction was developed and an exit interview conducted, underscoring the need for enhanced staff competency and documentation of medication‑related training.
