Park Terrace offers multiple dining venues to choose from so that the locale fits the residents’ mood. In the Main Dining Room, they serve breakfast, lunch, and dinner from daily menus.
Inspection Report Summary for Park Terrace
The California Department of Social Services conducted three unannounced annual inspections of Park Terrace, a 740‑type assisted living and memory care facility in Rancho Santa Margarita. Across all three visits—June 21 2021, May 31 2022, and June 12 2024—the evaluators noted that the facility maintained high standards of cleanliness, resident comfort, and regulatory compliance. The facility’s capacity of 230 residents, census hovering near 167–168, and hospice waiver for 15–20 residents remained within licensed limits, and staff consistently documented medication, safety, and health records in accordance with Title 22 Division 6 of the California Code of Regulations.
During the 2021 inspection, LPA Ruth Martinez observed an orderly environment: common areas, outdoor patio, and resident rooms all appeared clean and well‑stocked, and residents seemed happy and well cared for. The facility had fully implemented COVID‑19 precautions, including electronic screening, temperature checks, and adequate PPE supplies, and the emergency disaster plan and fire safety equipment were complete and functional. No deficiencies were cited, and the report was signed and acknowledged by Administrator Eugene Koehler.
The 2022 visit, led by LPAs Jessica Cho and Edward Tapia, focused on infection control. The facility again passed all safety checks; however, the evaluators identified a handful of minor items for correction, such as broken light bulbs in three apartments, a faulty grab bar in the elevator, and missing hand‑washing signage in resident bathrooms. An advisory note (LIC9102) was issued, and the administrator was asked to post larger COVID‑19 precaution signs and complete the requested repairs. Despite these recommendations, the report concluded with no formal deficiencies, and the administrator agreed to follow up on the corrective actions.
In 2024, LPA Ruth Martinez revisited Park Terrace, noting continued compliance and even expanded amenities such as a bistro and a swimming pool with a self‑latching gate. The facility’s food supplies, medication storage, activity programming, and safety systems remained fully functional. The evaluators reviewed resident and staff files, confirming required documentation, and verified that emergency drills and fire safety tests had been performed. No deficiencies were identified, and the report was reviewed with Executive Director Geno Koehler. The consistent pattern across all three inspections is a facility that maintains clean, safe, and resident‑focused operations while promptly addressing minor maintenance items when they arise.
