The complaint alleged that a resident diagnosed with Alzheimer’s and dementia (R1) sustained multiple falls that resulted in rib fractures, head injury, and pneumonia. The report documented three unwitnessed falls between 21 January 2022 and 24 January 2022, with delayed or incomplete assessment by medication technicians and caregivers. The facility failed to provide adequate fall monitoring, timely medical treatment, and sufficient staffing—particularly during weekends and evenings—violating CCR 87464(f)(1) and CCR 87705(c)(4). As a result, the department cited the facility, assessed a civil penalty, and required a Plan of Corrections by 20 May 2022.
